Fixed according to comments from Jesse.
v3-v4:
- Removed vport count.
- Optimized dpif_linux_port_add() for compatibility case.
v2-v3:
- Limit max number of ports to 64K.
- Limit actions-len to 16kb.
- Coding style fixes.

- Use hash table.
- Remove dpif-linux port number allocator.
