  MAINTAINERS: convert to .md format.

Most other doc files are in markdown format, so convert this one as
well.  Instead of linking to the web site, just use relative links
to the policy documents in the tree.

This patch also gives me a chance to hide a fix for my failure to
accurately sort names in alphabetical order.
